RN Service Line Coordinator: Full-Time Days

The RN Service Line Coordinator is responsible for determining the appropriateness of hospital admission, and for advocating, coordinating and facilitating the interdisciplinary plan of care to expedite medically appropriate, effective, efficient and timely utilization of resources for maximum patient outcomes.

The RN Service Line Coordinator partners with the UR Specialist, the Charge Nurse, the Social Worker, physicians and other members of the interdisciplinary team to facilitate safe and timely discharge, and intervenes as appropriate to remove barriers to efficient patient throughput and smooth patient transition.

The RN Service Line Coordinator applies clinical expertise and medical appropriateness criteria to resource utilization, admissions and discharge planning.

Qualifications

Minimum Qualifications

Education: Associate Degree in Nursing required. BSN preferred.

Licensure: Current Idaho RN license; compact license acceptable, must obtain Idaho RN license within 90 days of employment or current Idaho Social Work license.

Experience: Three years clinical experience in the acute care setting. Case management experience preferred.

Certification or Registration: Current American Red Cross or American Heart Association Basic Life Support or obtain within 7 days of date of hire. Certification in case management preferred.

Knowledge, Skills, Abilities: Ability to establish and maintain collaborative and effective working relationships. Able to assert needs to patients, families, physicians and other members of the interdisciplinary team while maintaining established rapport and relationships. Ability to communicate both verbally and in written forms. Strong critical, clinical thinking skills and the ability to assess and discern effective clinical care plans.
